The senior coordinator, person-centered services will provide case management and advocacy for individuals with disabilities supported by OBI and/or interested in exploring services with OBI. This person will help individuals and families navigate the “pathway” to services and becomes a critical member of an individual’s Person-Centered Planning Team. The senior coordinator, person-centered services will have extensive knowledge of OBI’s funding and referral sources (i.e., DDA, DORS, CCS’s, schools, etc.), including service definitions and requirements, and will establish and maintain excellent professional relationships with all stakeholders.
Essential Functions & Responsibilities
- Provide support to the manager of person-centered services by assisting with Person-Centered Plan (PCP) reviews and PCP approvals in LTSS.
- Assisting with auditing the PCP documentation to ensure compliance with requirements related to attendance and activities completed and progress towards goals.
- Provide case coordination services to the participants supported by OBI by ensuring meaningful programing and the highest quality of care and support through the person-centered planning (PCP) process.
- Work collaboratively with Coordinators of Community Services (CCSs) to facilitate timely Person-Centered Planning (PCP) meetings.
- Work with each participant and their team to discuss and establish person-centered goals to be included in the Service Implementation Plan (SIP).
- Maintain close communication with OBI direct support professionals to assist with program planning and monitor the implementation and progress toward PCP goals.
- Develop and submit all required documentation to request and secure appropriate DDA funding for each individual participant.
- Monitor participant documentation within OBI’s online database platform for accuracy, quality, and to remain up to date on each participant’s progress.
- Involve OBI’s Workforce Development team whenever a participant is interested in employment services.
- Involve OBI’s Manager, Personal Supports whenever a participant is interested in Personal Supports services from OBI.
- Review and approve required documentation on time, including but not limited to OBI’s online database platform service and attendance logs.
- Review PCPs on LTSS for accuracy and provide feedback to CCS and/or Manager of Person-Centered Services regarding any necessary changes that need to be made before services can be accepted.
- Review and authorize services for participants in LTSS.
- Assure Behavior Plan compliance by assisting the Behavior Specialist.
- When requesting/advocating for individual services, provide DDA and/or the CCS with relevant and substantial back-up documentation and data to justify the request.
- Train staff appropriately for each participant on caseload that requires specific training and maintain required documentation that the training occurred.
- Example: Specialized protocols for feeding or toileting, documented in the PCP.
- Work with OBI’s managers and the person-centered planning team to assure DSPs are trained in the individuals' specific trainings.
- Maintain accurate and complete records for each participant as instructed in the client management system.
- Follow all DDA and OBI regulations when investigating, monitoring, and reporting incidents.
- Assist to assure quality services and agency regulatory compliance.
- Provide back-up transportation as needed for participants.
- Provide Direct Care as needed for participants.
- Communicate with families/caregivers, and CCSs on a regular basis, being sure to communicate any changes or important information with the entire team.
- Attend regular departmental meetings.
- Intervene in crisis situations to assist participants in resolving time-sensitive issues; as well as ensuring the appropriate services are received.
- Establish relationships with other appropriate community nonprofits and service providers, to collaborate and share resources.
- Maintain up-to-date knowledge of DDA funding, service definition and regulations.
- Maintain compliance with all OBI, DDA, OHCQ, and DORS policies and regulations at all times.
- Maintain the participant files on the server.
- Submit all OBI forms to the CCS as part of the PCP.
- Provide assistance with virtual support as needed. Including but not limited to the AM and PM Zooms.
- Attend external meetings, conferences and trainings as requested by supervisor.

Qualifications- Minimum of 1 year of successful experience working with individuals who have disabilities, providing case coordination/case management, or serving in a quality assurance or compliance capacity.
- High school diploma or equivalent required; bachelors' degree in a related field preferred.
- Excellent written and oral communication skills.
- Highly organized and demonstrated ability to work well independently and as part of a team
- Experience in using a client management system; I Care Manager preferred.
- Experience in LTSS and a knowledge of the DDA service definitions preferred.
- Valid Driver’s license (no more than 3 points) and 3 years driving experience.
- Ability to perform and meet job and physical requirements.
- Proficient in Microsoft Office and a broad understanding of (or ability to learn about) technology that supports community development and person-centeredness and/or virtual service delivery, including apps such as Zoom and Google Voice.
