Open Door Family Medical Centers is seeking a bilingual (Spanish) Registered Nurse (RN) to join the busy and dedicated School Based Health Centers (SBHC) program in providing quality care to students in the Port Chester Schools. This position will promote and restore patients health by completing the nursing process, collaborating with the Nurse Practitioners and Patient Care Technicians to provide comprehensive quality care and support to patients and families. He/she will participate in cross-functional care teams, typically working the standard SBHC hours of 8am to 4pm, Monday through Friday. Responsibilities: Performs nursing duties in accordance with NYS Education Practice Guidelines. The Registered Nurse (RN) will promote and restore patients health as they provide, delegate, evaluate and coordinate comprehensive nursing care through the use of the nursing process for patients of all ages, gender, cultures, and background; provide physical and psycho-social support to patients and family members. The RN, School Based Health Centers, provides care to patients of pediatric and adolescent age groups and have responsibilities in all School Based Health Centers located in Ossining and Port Chester, NY. PATIENT CARE SERVICESAssesses, plans, directs, and evaluates nursing care as determined by the patients age related physical, psychosocial, and cultural needs.Performs and records screening tests (blood pressure, temperature, pulse, weight, EKG, vision, hearing) to patients of pediatric and adolescent age groups.Administers medications, vaccinations, treatments, and intravenous solutions; perform lab/diagnostic testing and therapeutic procedures as per standing orders/ or written order by the clinician. Safely stores and maintains inventory of medications, immunizations in accordance with the CDC and NYS Vaccine ProgramMaintain up to date immunization record through NYSIIS, follows current ACIP guidelines, and participates in the storage and maintenance of medications and immunizations.Reviews patients medication list, communicates patient status and needs to the provider prior to the patient visit for all patients, highlighting chronic disease and high-risk patients.Maintains emergency equipment and oxygen tanksCollects and performs CLIA laboratory procedures including but not limited to rapid strep test, urinalysis, pregnancy screening, hemoglobin A1C, and blood glucose monitoring pregnancy tests. Records and reports result appropriately, calibrates, instrument maintenances and cleans following manufacturers instructions.Prepare and maintain documentation in compliance with JCAHO standards and applicable city, state, and federal laws.Performs monofilament diabetic foot assessment and records results CARE COORDINATION AND COMMUNICATIONThe RN collaborates with the team; delegates follow up of patient care to PCTs by ensuring that laboratory reports, consultants report and diagnostic reports, discharge and ER visits are up to date in the patients file before the next visit. Handles telephone encounters and disseminates communication to team members and patients.Monitors the Relevant Dashboard and implements outreach process to recall patients to address care gaps; participates in pre-visit planning and daily team huddles.Reinforces health education and counseling, including general health maintenance; birth control; pediatric care; nutrition; asthma care; STD prevention and treatment; HIV testing and counseling; alcohol, drug, and domestic abuse awareness; and smoking cessation.Participates in quality improvement initiatives as part of the practice care team. SUPPORTING ROLEAssist care team in scheduling and follow-up of missed appointments and non-complaint patients in person and by telephone and mail.Participates in training, orientation, competency assessment and education of new employees.Communicates clearly, concisely, and accurately, creating a working climate that provides growth and job satisfaction of personnel. Demonstrates clinical competency.Serves as a resource person to other staff in the area of ones own clinical expertise.Maintains infection control and safety practices in accordance with CDC, OSHA Standards.Participates in back-to-school nights, health fairs and other outreach events to promote health and enroll new patients in the School Based Health Centers.
