Are you a detail-oriented and experienced healthcare professional with expertise in MDS coordination? Join our team as a Regional Mobile MDS Coordinator, where you’ll play a pivotal role in ensuring accurate and timely Medicare/Medicaid documentation for our facilities.

What You’ll Do:
As the Regional Mobile MDS Coordinator, you will:

• Ensure the accurate and timely completion of all MDS Assessments, including PDPM Medicare and OBRA assessments.
• Conduct ongoing MDS reviews to optimize Patient-Driven Payment Model (PDPM) categories and achieve maximum allowable reimbursement.
• Coordinate interdisciplinary participation in the MDS process and integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services to ensure accurate assessments and reimbursement.
• Manage MDS documentation schedules, data entry, and electronic submissions, ensuring compliance with federal and state regulations.
• Lead weekly Medicare/CMI meetings and collaborate with teams to enhance care plans based on resident needs.
• Oversee resident care conferences, review quality measures, and audit documentation for accuracy.
• Train and mentor new MDS coordinators to uphold high standards across all facilities.

What You Bring:

• Licensure: Current and unencumbered Registered Nurse (RN) license.
• Experience: Long-term care and MDS coordination experience preferred.
• Skills: Strong organizational and communication skills with the ability to read, write, speak, and understand English fluently.
