JOB SUMMARY
The Phone Triage Registered Nurse is responsible for managing all aspects of the staffing process based on triaged calls. Responsibilities include answering phone calls from the call center, gathering further information, providing education if appropriate, and triaging situations based on acuity and scheduling visit nurses as appropriate. This position also requires establishing and maintaining positive relationships with all related entities (internal staff, clinical field employees, contract staff, patients/families, referral sources, etc), responding to staff requests/concerns, and maintaining information to assist in the coordination of patient services and staff availability.

DUTIES & RESPONSIBILITIES
- Serves as primary recipient to all inbound phone calls from the call center
- Return calls to patient/family/caregiver/referral source/etc. to provide education, gather further information, and formulate a plan for monitoring or visit.
- Contacts visiting clinicians to discuss visit needs and writes orders/assigns visits as appropriate and needed.
- Understands and accurately completes intake process for referral, insurance, and authorization entry.
- Assists the Intake and Scheduling department by supplying staffing solutions to new patient referrals and completes/schedules referrals to fill gaps in weekend scheduling.
- Accurately completes all workflow associated with staff requests for scheduling changes, caseload reassignments, recertification/discharge of services, new referral scheduling, and missed visits. Accurately looks ahead to fill gaps in scheduling caused by missed or returned visits.
- Communicates effectively, professionally, and thoroughly with staff regarding coordination of care expectations, educates and enforces deadlines, and establishes and maintains positive working relationships with current staff and contract staff.
- Provides insight to identify needs for additional staff

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
- Must have completed a requisite practical nursing program and be currently licensed as a Registered Nurse in the state where the agency is located.
- Three years nursing experience preferred. Home Health and/or hospice experience preferred.
- Complies with accepted professional standards and practice.
- Demonstrates strong written and oral communication skills in person, over the phone (verbal and text) and over email.
- Provides excellent customer services skills with productive organizational skills and habits.
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
- Technical aptitude to know, learn, and master the agency's software.
