Basic Function
Provide assessments, evaluation, planning, and community support of assigned LTS program services. Determine eligibility for PCA services and other in-home supports. Teach and provide skills training to care attendants on topics/skills that will help them be successful with their employment and provide skills training to consumers on a variety of topics that will empower and promote independence. Review consumer's care plans to ensure care needs are being met and adjust as needed. Assist in obtaining services which will enhance the consumers quality of life and promote independence. Assure all paperwork is completed in accordance with GSIL policy & procedures. Assist in outreach efforts. Serve as community I & R for Consumer Directed care options. Travel required.
Responsibilities
- Participate in the assessment and evaluation process for eligibility in GSIL's consumer directed in home support programs.
- Consult with individuals about health care concerns and provide information on available options.
- Consult with other staff regarding appropriateness of an individual's participation in attendant care programs.
- Provide orientation and skills training to consumers regarding attendant care program policies and procedures.
- Address health care management issues and emergency medical planning. Request and participate in risk assessment meetings.
- Monitor participants' care plan on a Quarterly basis and more often per regulations/ as the consumer's needs change, including 6 month and annual reassessments. Provide ongoing support and skills training as needed.
- Coordinate services and keep and maintain accurate records for consumers. Assure consumer records are maintained in accordance with GSIL policy and procedures.
- Serve as an information resource for community and staff on personal care options, community supports, transitions, health and disability related issues
- Manage an ongoing case load of PCA/NF Tran/and or PCSP consumer directed services.
- Conduct other duties as assigned that will increase options for persons with disabilities to live independently in the community.
- Work collaboratively with care managers from the MCO's and CFI case managers to address the consumers care needs.
- Keep ACE recruitment requests accurate and up to date.
- Participate in the presentation and teaching of the department trainings.
- Assist consumers with Nursing Facility Transitions and monitor status on an ongoing basis.
- Instruct/assist consumers on ways to hire new ACE's, assist ACEs with online application and other mandatory forms, review paperwork for completeness.
- Care Coordination
- Participate in committees as assigned.
- Conduct ACE training classes as scheduled.
- Work a flexible work schedule in accordance with department needs
- Maintain professional knowledge/skills by reviewing professional literature and attending training/workshops.
- Support and uphold GSIL policies and procedures, workplace regulations and safety practices.
Education/Training/Experience
Registered Nurse with current New Hampshire License and two years of experience working with individuals with long term health care needs. Must have a commitment to facilitating and maximizing independence for persons with a disability or seniors with chronic health care needs.
