About The Position
**Job Title:** Registered Nurse (RN) - Home Care **Job Specialty:** Home Care **Job Duration:** 13 weeks **Shift:** 8-hour days, 8:00 am - 4:30 pm, Monday to Friday; on-call weeknights and weekends on a rotational basis **Guaranteed Hours:** 40 hours per week **Experience:** Minimum of 1 year as a Home Health RN **License:** Current State of Illinois RN license **Certifications:** BLS **Must-Have:** - Patient assessment skills - Medication management - Wound care expertise - IV competency with PICC and peripheral lines - Competence in lab draws, foley and suprapubic catheter care, and ostomy care - Strong communication skills - Ability to educate patients and families **Job Description:** - Independently see and assess an average of 5 patients per day - Perform patient education and head-to-toe assessments, including vital signs and breath sounds - Conduct skin/wound assessments and carry out dressing changes and wound care, including Wound Vac - Provide PICC line care, perform lab draws, manage foley catheter changes, and care for ostomies - Manage medication reconciliation and education - Document appropriately using Epic - Communicate effectively with providers and care team - Coordinate patient interdisciplinary care with providers, therapy staff, social workers, wound centers, and infusion departments - Use equipment such as laptops, portable printers, and cell phones for communication **Training and Orientation:** - Extensive 1:1 staff development and training on wound care, IV class, online modules - General orientation lasting approximately 5-7 days **Additional Information:** - Accepts local candidates within a 50-mile radius - Dress code requires professional-looking scrubs - Drug-free workplace; negative drug screen required This job posting presents an excellent opportunity for RNs passionate about home care and patient education.
