Responsibilities:In keeping with the Dignity Health core values the Resident Assessment Coordinator assists the Nursing Manager of SAU in achieving institutional goals and coordinates an interdisciplinary approach to the implementation through completion of the Minimum Data Set.
This position is represented by SEIU Local 121RN and is covered by the terms and conditions of the applicable collective bargaining agreement.
Qualifications:Minimum Qualifications:
- Current California RN license
- Current CPR certification.
- Recent experience in long-term care, medication and treatment administration, and the mandated OBRA regulated Minimum Data Set, Resident Assessment Protocol summary and care planning process.
- Computer literate.
- Communicates clearly, concisely, and accurately with people, verbally and in writing.
- Excellent communication and interpersonal skills and the ability to work with various publics and interdisciplinary teams.
- Excellent organizational skills.
- Thorough knowledge of the MDS and RAP protocols.
