Position Summary
We are seeking a dedicated and compassionate RN Case Manager to join our team. In this role, you will provide high-quality home health care, coordinate patient services, and ensure the best possible outcomes for patients and their families.
Key Responsibilities
• Conduct initial evaluations and assessments for patients.
• Develop, implement, and revise individualized care plans.
• Complete OASIS assessments at appropriate time points.
• Regularly re-evaluate patient needs and adjust care plans accordingly.
• Prepare clinical and progress notes while ensuring proper documentation.
• Coordinate services and make referrals as needed.
• Participate in discharge planning and ensure continuity of care.
• Maintain compliance with professional standards and best practices.
• Communicate effectively with patients, families, and interdisciplinary teams.
• Must be a licensed driver with a reliable, insured vehicle.
Education & Qualifications
• Licensure: Must hold a current Connecticut RN license.
• Certification: CPR certification required.
• Experience: Home health care experience preferred.
• Skills: Strong communication, organization, and time-management skills.
Schedule
• Day, evening, and weekend shifts are available.
• On-call duty as scheduled.
Compensation & Benefits
Competitive Pay
• Base Salary: $38.00 - $45.00 per hour (negotiable based on experience).
• Full-Time Salary: Based on a 40-hour workweek, paid weekly.
• Orientation Pay: $25.00/hour until signed off for independent work.
• Per Diem Visits: Additional pay for visits exceeding the weekly quota.
• On-Call Stipend: $150 per week when assigned as the on-call RN.
Additional Benefits
• Mileage Reimbursement: $0.50 per mile.
• Paid Time Off: 4 weeks, accrued weekly, plus 8 hours of float holiday.
