About The Position
**Job Title:** Registered Nurse **Job Specialty:** Case Manager **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (8:00 AM - 4:30 PM) **Guaranteed Hours:** Not specified **Experience:** 2 years minimum experience required; screen if less **License:** Must have DC license in hand upon submission **Certifications:** BLS required **Must-Have:** - Acute Hospital experience - Long Term Acute Care/Rehab/Skilled Nursing experience - Knowledge of Admission Criteria, Care Coordination, and Discharge Planning - Ability to Utilize InterQual Criteria and Milliman Guidelines - Previous Charge Experience Preferred - 1 prior travel assignment; screen if less **Job Description:** - Conduct comprehensive assessments and develop individualized care plans addressing Social Determinants of Health (sDOH) needs. - Collaborate with hospitals, physicians, and other healthcare providers to ensure successful execution of care plans. - Documentation must reflect NCQA and contract requirements/timelines. - Care for a diverse age group from Pregnant Persons/Newborns to Geriatric patients within the Medicaid recipient, vulnerable population. - Knowledge of regulatory skills including CMS, CPT coding and billing, Department of Health, DRG, HEDIS Measures, HIPAA, ICD-10 Coding, NCQA, OSHA, The Joint Commission/Core Measure/National Safety Goals, and Worker’s Compensation. - Must be able to address chronic diagnoses like High Risk Pregnancy, Postpartum, DM, CHF, COPD, Asthma, obesity/overweight, and all Behavioral Health. - Office work setting in Washington DC; some face-to-face required at the wellness center. - Familiarity with EMR systems (e.g., guiding care/Healthedge). - Ensure all employees receive the flu vaccine. - Be prepared to potentially come into contact with COVID Patients. - Follow office dress code; Business casual. - Participate in required training and orientation; 28 hours.
