Job Description:
As an RN Care Manager, you'll play a pivotal role in engaging with our members, either in community settings or through telephonic interactions, to conduct thorough assessments and develop tailored care plans collaboratively. Utilizing motivational interviewing, active listening, and problem-solving skills, you'll empower our members to take charge of their health and well-being.
Primary Responsibilities:
Engage with members either in community settings or telephonically.
Conduct comprehensive member assessments, considering bio-psychosocial, functional, and behavioral health aspects.
Utilize motivational interviewing techniques to assist members in identifying and achieving their intrinsic goals.
Employ active listening to gather and retain crucial member information during assessments.
Collaborate with members to develop individualized care plans.
Identify and escalate urgent member situations promptly.
Assist members in setting healthcare goals and overcoming barriers to achieve them.
Educate members on health and wellness to facilitate self-management.
Connect members with relevant community resources.
Present complex cases to an interdisciplinary care team for comprehensive input and guidance.
Utilize computer applications, such as Excel, Word, and Outlook, to summarize and communicate member information effectively.
Document assessments, interactions, referrals, and follow-up plans meticulously in relevant systems.
Stay abreast of changes to computer systems and apply this knowledge effectively.
Achieve performance goals within a virtual environment.
Required Qualifications:
Reside in the Philadelphia, PA region.
Bachelor's degree or Associate degree with RN license.
Unrestricted license as a Registered Nurse in the state of PA.
4+ years of experience in Case Management and/or Care Coordination.
Experience managing populations with complex medical or behavioral health needs.
Proficiency in Microsoft products, including Excel and Outlook; ability to quickly learn clinical applications.
Designated workspace and secure high-speed internet via cable/DSL at home.
Current driver's license, auto insurance, and ability to travel for occasional field visits.
Strong passion for assisting members with complex needs.
Preferred Qualifications:
Background in Managed Care.
Certified Case Manager (CCM) certification.
Bilingual capabilities a plus.
Experience in utilization review, concurrent review, and/or risk management.
