About the Job
Personalized Health Partners, is currently recruiting a Registered Nurse for the position of RN Case Manager.
THE ROLE
The RN Case Manager assists with implementation of strategic network initiatives, supports growth our of innovative healthcare practice, and provides high quality, value based care to the senior patient population within our network.
Our network consists of a variety of payors, an accountable care organization (ACO), telemedicine, and Medicare advantage products. The RN will assist the providers and the practice with services to maximize quality care and the customer experience. The RN will maintain frequent contact with our patients either face to face, telephonically, or virtually to provide education and clinical assessments to ensure high patient adherence to treatment plans that will impact positive clinical outcomes.
EDUCATION and QUALIFICATION:
Required:
- Nursing degree, Bachelor’s of Science or Associate of Science is acceptable
- Active, unrestricted state license
- Ability to travel locally if needed
- US work authorization
- 3+ years experience as a nurse
- Ability to work in a flexible environment and schedule
- Passionate about providing healthcare to the senior and low-income populations
Preferred:
- Training or experience in geriatrics such as skilled nursing centers, primary care setting, or assisted living facilities
- American Heart Association CPR Certification or willing to be certified
- Management or experience working in a metrics, results driven healthcare model such as case management for insurance company or accountable care organizations (ACO)
- Be open to professional development through training, obtaining certifications if necessary, and attending team meetings
- Experience working in a collaborative healthcare setting to drive positive outcomes and achieve goals
- Training or experience in outpatient primary care setting
JOB SKILLS:
- Must have strong oral and written communication skills
- Must be detail oriented with an ability to work well both independently and in a team setting
- Exhibit a customer service approach with teams and patients
- Strong time management skills required. Must be able to prioritize and adhere to competing deadlines while achieving goals
