Overview:
Coordinates and ensures effective supervision of a cluster of hubsites in the Community Hubsite Program.
Responsibilities:Primary duties and responsibilities include but not limited to:
- Supervises Site Managers through frequent contact (phone; e mail; meetings, etc.) and scheduled/unscheduled visits at least weekly to ensure the overall effectiveness of each hubsite.
- Reviews regulatory and fiscal documentation as needed to ensure adherence to agency policy and procedures. Ensures that all individuals’ program plans and related documentation adheres to OPWDD and HCBS Waiver documentation.
- In coordination with Site Managers and administration, ensures that appropriate staff candidates are selected for vacant positions in the hubsite program.
- Participates in supportive supervision, including formally addressing performance issues as needed, and ensuring that all associated documentation is provided to supervisor and/or Human Resources as per agency and program policies and procedure.
- Ensures maintenance of COMPASS standards and requirements through direct supervision of hubsite program practices and operations.
- Provides training to Site Managers, and other hubsite staff as needed in areas related to Agency/Program policy & procedures, COMPASS, regulatory and fiscal requirements, and general hubsite operations.
- Ensures and/or provides adequate direct support and assistance to people receiving services during programming hours and/or during times of transportation.
- Confers with clinical, MSC, residential, families, providers as needed through attendance at scheduled meetings and all modes of contact in order to formulate and disseminate individual-specific information; facilitate program planning and ensure optimum services for all individuals.
- Ensures the accuracy of financial records by reviewing and signing documentation related to financial transactions such as: petty cash, hubsite purchases and transactions.
- Serves as a member of the Interdisciplinary Treatment Team by participating as needed in ISP and all Special I-Team meetings.
- Works in conjunction with administrative and supervisory staff to establish and ensure adequate staffing coverage daily for community hub site program.
- Ensures that Site Managers adhere to confirmed staffing patterns by reviewing all payroll information (time sheets), staff attendance logs, and vacation requests.
- Ensures that ongoing communication exists between the Community Hubsite Program and the individual’s residence (family or provider) through scheduled meetings, all modes of contact to ensure coordination of services.
- Assists all Hubsite staff by remaining fully accessible (via cell phone) during program hours.
- This position requires regular and substantial unsupervised and unrestricted physical contact with people receiving services and, therefore is subject to successful completion of fingerprinting/criminal background check.
- Ensures that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.

Qualifications:
- BA or BS in Human Services or related field required
- Qualified NYS Driver’s license
- Minimum three (3) years’ experience supporting people with intellectual and other developmental disabilities.
- Prior supervisory experience preferred.
- Computer literacy.
AMAP, SCIP Certification & CPR Certification must be obtained within 4 month orientation period.
