Qualifications:
- MD/DO degree
- Board Certified family practice, internal medicine or emergency medicine physician
- Active and unrestricted medical license in New York
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal abilities
- Experience in post-acute care a plus
