Overview

Are you a Physical Therapy Assistant (PTA) with a desire to make a difference in the lives of the older adult population? If so, we sure have the job for you! Come join our EMPLOYEE-OWNED TEAM! and make a difference in at Common Heart HHC in Beliot, WI area as a Full-Time Physical Therapist in Home Health Care.

This is a PRN Home Health position to cover nearby cities within a 30 mile radius. Mileage reimbursement included.

Responsibilities

As a Physical Therapist Assistant (PTA), you will work closely with the Physical Therapist to establish appropriate goals and creatively execute the treatment plans. As a member of our interdisciplinary team, you will be required to maintain productivity standards and complete required documentation. More importantly, you will help your patients achieve critical outcomes while acting as a trusted partner to the community you serve.

Qualifications

• Be a graduate of an accredited school for Therapists.
• Hold a valid or pending (for newer graduates) state license.
• Acute care, long-term care and/or rehabilitation experience preferred
