Allegria Village is hiring for a Per diem/Contingent Physical Therapist in our Skilled Nursing Facility.
Education/Certification:
• Graduate of an accredited Bachelors, Masters, or Doctorate in Physical Therapy.
• Current license from the state Board of Physical Therapy Examiners.
• Graduate of an accredited Bachelors, Masters, or Doctorate in Physical Therapy.
• Current license from the state Board of Physical Therapy Examiners.
JobResponsibilities:
1.Providesdirectpatientcare,evaluatesfunctionalneedsandoutcomes,andconsultswithother specialists.Reviewspatients’functionalneedsandadjustscareplanstoensureoptimalpatient outcomes.
2.Designsandimplementsaplanofcareforpatientsbasedonathoroughassessmentofpotential developmentandincorporatesparticipationbysignificantotherswheneveritseems appropriate.
3.Conductstherapypracticewithindefinedstandardsofcare.
4.Directstheassistanceofsupportstafftoachievepatients’functionalgoals.
5.Communicateseffectivelywithotherprofessionalandsupportstafftoachievepositivepatient outcomes.
6.Practicesfacilityanddepartmentalpoliciesandprocedures,objectives,qualityimprovement program,safety,environmental,andinfectioncontrolstandards.
7.Documentsevaluations,treatmentgoals,andplans.Regularlyupdatesdocumentationand maintainsprescriptionsandsignatures.
8.Enhancesprofessionalgrowthanddevelopmentofselfandothersthroughparticipationin educationalprograms,in-servicemeetings,andworkshops.Keepsuptodateonprofessional literature.
9.Maintainsa positive working relationship with other health professionals and ancillary staff.
10.Otherdutiesasassigned

Qualifications:
•KnowledgeandskillstocareforpatientneedsoftheSNFpopulation
•PriorSkilledNursingFacilityexperiencepreferred
•Proficiencywithallapplicablestate,federal,andprofessionalstandards
•Mustpossessproblemsolvingskillstosuccessfullyperformjob
•Excellent verbal and written communication skills.
•Excellent organizational skills and attention to detail.
