Pay-Per-Visit (PPV) position

Job Title: Home Health Physical Therapist
Location: Somerset, PA
Territory/Driving Expectation: Somerset, Donegal, and nearby areas
Schedule: M-F 8am-5pm; 1 weekend/month; 2 holidays/year
Travel: Local travel to patient homes

Key Responsibilities:

• Conduct patient care in home settings across assigned territories, adapting treatment plans to meet individual needs.
• Collaborate with the interdisciplinary team to develop, implement, and adjust patient care strategies.
• Efficiently document patient progress using Electronic Medical Records (EMR) systems, including Practice Partner EMR.
• Engage in ongoing professional development and mentoring through clinical educators and supervisors.
• Manage scheduling and patient visits with flexibility and autonomy supported by dedicated scheduling coordinators.

Required Qualifications:

• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Active Physical Therapist license in Pennsylvania.
• Minimum of one year of healthcare experience.
• Proficiency in handling Electronic Medical Records (EMR) and mobile technology, including tablets and smartphones.
• Valid driver’s license and ability to travel for patient visits (approximately 20% of your day).
• Strong self-management skills for time and schedule organization.
