Position Summary
To provide physical therapy services in complies with laws and regulations applicable to this position and act in accordance with Corporate Compliance Program. The Physical Therapist is to perform patient care tasks under minimal supervision with advanced skills in clinical reasoning, problem-solving and treatment interventions. Evaluate and treats patients, as well as provide direction and supervision of Physical Therapist Assistants as indicated within the Texas licensing Board and American Physical Therapy Association scope of practice.
Essential Duties and Responsibilities
Provides direct customer care that meets department and professional standards of practice, including patient evaluation and assessment, development of physical therapy care plan, patient treatment, family/caregiver training and discharge planning. May serve as a clinician in both the inpatient and outpatient settings
Examines, evaluates, and tests customers with mechanical, physiological and developmental impairments, functional limitations, and disability or other health and movement-related conditions. Determines diagnosis of the physical impairments or movement-related functional limitations, prognosis, plan of therapeutic intervention and frequency/duration of intervention
Designs, implements, and modifies therapeutic interventions to alleviate movement-related impairments, functional limitations and disabilities. Selects and administers appropriate assessment and treatment techniques
Evaluates for, recommends, applies and, as appropriate, prosthetic and supportive devices and equipment, and trains patients in their use
Participates in clinical teaching, including the clinical instruction of physical therapy interns/students and other team members. Provides input regarding the educational needs of team members and serves as a resource in meeting those needs. Provides mentoring to others when appropriate and serves as a role model/resource/teacher/mentor to less experienced Physical Therapists
Performs all necessary customer care documentation, including assessment, plan of care, progress notes, discharge summaries, and charges for evaluation and treatment. Documentation reflects customer status, progress or change, participation by customers and family in goal setting and delivery of care
Discharges customer from treatment when appropriate with adequate discharge planning including equipment procurement and family training, and communicates this appropriately to Recovery Care Coordinator
Schedules and maintains assigned customer caseload consistent with department productivity standards. Informs supervisor if problems arise related to the delivery of customer care or caseload volume. Ensures documentation and billing are timely, accurate and complete, including evaluations, re-certifications, progress notes and discharge summaries
Initiates and participates in interdisciplinary care planning to meet the needs of each customer and family, including participation in rounds and customer care conferences, communicating with and teaching other care providers. May contact community agencies as necessary to provide continuity of service between PureHealth and community
Identifies needs for equipment repair or upgrade to maintain quality of care. Makes recommendations for selection and purchase of specific equipment and supplies.

Work Environment
This job operates in a post-acute setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facility’s no-smoking policy.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 60 pounds.
Position Type and Expected Hours of Work
This is a position in a post-acute setting that is open 24 hours a day, 365 days a year. Weekend and holiday shifts will be required.

Required Education/Experience
Graduate of an accredited therapy program
Required License/Certification
Valid State Appropriate PT License
Preferred Education/Experience
A minimum one (1) year of related experience
