Key Responsibilities:
- Provide direct rehab care in physical therapy upon physician referral, following all applicable laws, regulations, and company standards.
- Conduct comprehensive patient evaluations to determine therapy needs.
- Develop and implement individualized treatment plans in collaboration with interdisciplinary teams.
- Administer therapeutic exercises, manual therapy, and other modalities.
- Educate patients and families on treatment protocols and at-home exercises.
- Oversee and evaluate care provided by PTAs, Rehab Aides, and PT and PTA students.
- Maintain accurate and timely documentation using therapy software.
- Ensure compliance with federal, state, and company standards and regulations.
- Participate in community health projects as appropriate.
Qualifications:
- Graduate of an accredited program in physical therapy (BSPT, MSPT, or DPT).
- Current and active Physical Therapist license in Massachusetts, maintaining it in good standing.
- Preferably one year of experience in post-acute care or a related setting.
- CPR certification upon hire or during orientation, maintained throughout employment.
- Proficiency in Microsoft Word, Excel, and email.
- Strong interpersonal, communication, and independent decision-making skills.
- Knowledgeable of post-acute care regulations and guidelines.
- Commitment to patient-centered care and professional integrity.
Essential Functions:
- Follow and adapt physical therapy treatment plans for patients.
- Establish, assess, and modify realistic, measurable, timely, and functional goals.
- Provide excellent customer service with a positive attitude.
- Assist in patient evacuations if necessary.
- Demonstrate dependable attendance and effective reasoning and judgment skills.
- Communicate and function effectively within an interdisciplinary team.
- Perform physical tasks such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, and reaching during working hours.
- Proficiently read, write, speak, and understand English.
