Responsibilities
**General Purpose:**
Clinical staff pharmacists are responsible for the provision of comprehensive clinical pharmacy services for Medical/Surgical adult inpatients to optimize safe and effective drug therapy. Clinical activities include: performing medication order verification, pharmacokinetic drug monitoring, other patient acuity monitoring; providing drug information, and participating as a member of an inter-disciplinary patient care team. The clinical staff pharmacist will also have a working knowledge of the operational aspects of the department and will assist with dispensing medications and providing technician oversight, when applicable.
**Responsibilities:**
+ Understand and follow departmental and hospital policy and procedures related to accurate medication order verification and dispensing of medications
+ Interact with providers, nurses, and other healthcare professionals to guide and recommend appropriate medication use based on patient-specific information
+ Participate in focused clinical initiatives as defined by the department or team including pharmacokinetic consultation/monitoring, antimicrobial stewardship, interdisciplinary rounds, and other patient acuity monitoring activities
+ Proactively identify and resolve patient care issues and problem medication orders, document clinical interventions, and communicate appropriate handoff at change of shift
+ Perform duties of pharmacist in charge (when assigned), including coordination of shift resources, directing and supervising the work of technicians, problem solving, and staff conflict resolution – contact supervisor for assistance when additional expertise or authority is needed
+ Participate in the education and mentoring of other pharmacy staff or learners (newly hired pharmacists, residents, technicians, students)
+ Identify the need for patient education and perform such tasks, as able
+ Participate in departmental Q/A processes such as reporting and resolving medication adverse events, near-misses, or errors
+ Volunteer to participate in root-cause analyses and medication use evaluations, as able, and assist with revising hospital or departmental policies, procedures, treatment guidelines, or order entry functionality, when needed
**Qualifications:**
Required:
+ Bachelor of Science degree in Pharmacy or Pharm D preferred.
+ New York State Pharmacy License or eligibility
Preferred:
+ Either prior (>1 year) relevant acute care pharmacy experience or has successfully completed an accredited pharmacy practice residency
+ Strong problem-solving and communication skills with an interest in acute care pharmacy practice. Training in hospital pharmacy practice, including internship.

**Pay Range**
Pay Range: $107,910 - $161,907 Annually
_The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._
