We're seeking a dynamic Pharmacist to join our team and make a difference in the lives of residents in long-term care facilities.
The daily function of a Pharmacist is to review, enter, and clarify nurse/ MD questions regarding prescriptions for residents of a long-term care facility. Prepares medications by reviewing and interpreting physician orders and detecting therapeutic incompatibilities. Dispenses medications by compounding, packaging, and labeling pharmaceuticals. Controls medications by monitoring drug therapies; advising interventions.
Location: Worthington, OH
Salary range: $55-$65/hour
Schedule: Rotating shifts and every 4th weekend OR Monday – Friday 3:00 pm – 11:30 pm

Key Responsibilities:
- Enter and review faxed, verbal, electronic, and paper orders.
- Initial Review orders (PV1) and Final Review Orders (PV2).
- Contact physicians for prescriptions when appropriate.
- Speak with providers regarding prescription issues and clarifications.
- Enter, check, and replace contents of EKits and Automated Dispensing Machines (ADS).
- Pull and log-controlled substances.
- Process new and completed backorders.
- Enter and compound IV/TPN and non-sterile compounds.
- Make necessary NDC changes.
- Check pre-packed medications.
- Process OOP/LOA requests as they come in.
- Triage documents to the appropriate queue.
- Complete Quality Assurance (QA) sheets as necessary.
- Process orders for REMS medications (i.e. clozapine).
- Making clinical decisions and performing clinical reviews of patient profiles.
- Perform on-call duties as scheduled.
- Knowledge of medications, interactions, and dosage.
- Report medication errors as they occur
- Receive and enter 222 forms from facilities.
- Understand billing procedures and communicate with the billing department.
- Perform pharmacy opening and closing functions.
Qualifications:
- Licensed by the State Board of Pharmacy (must be in good standing)
- 2-3 years of experience preferred
- LTC Experience Preferred
- Prior experience in long term care and with Frameworks and Docutrack information systems is strongly preferred
Skills & Abilities:
- Must be able to communicate effectively both verbally and in writing
- Must possess attention to detail and organizational skills
- Must be able to adhere to regulatory compliance policies and procedures
- Must possess time management skills to meet deadlines
- The ability to adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer
