Clinical Staff Pharmacist
General Summary:
Collaborates with patients, physicians and other health professionals to ensure the responsible provision of drug therapy.
Essential Job Functions:
Oversees the drug distribution system to assure that medications are available for the patient when needed.
Is knowledgeable about and complies with hospital and departmental policies, procedures, regulations and standards as related to job.
Maintains records of drug distribution activities in compliance with Hospital policy and state and federal law.
Supervises Pharmacy Technician activities related to medication preparation and dispensing.
Is knowledgeable of and compliant with sterile compounding regulations (e.g. USP 795, USP 797, and USP 800).
Reviews all provider orders that pertain to drug therapy for inpatients.
Works collaboratively with providers and other care-givers to optimize medication therapy (e.g. drug choice, dose, cost effectiveness, avoidance of drug duplications, interactions & ADRs).
Serves as a drug information resource for caregivers.
Provides clinical services per department policies. These include consulting on antibiotic and anticoagulation therapy, renal impairment dose adjustments, parenteral nutrition formulations, IV to oral substitutions, therapeutic interchanges, and reviewing medication reconciliation on admission and at discharge.
Serves as a preceptor for pharmacy students.
Participates in discharge planning meetings.
Provides medication information & education to patients as requested.
Is an active member of interdisciplinary committees and/or educational teams.
Participates in quality assurance activities and Pharmacy initiatives.
Maintains competencies necessary to efficiently use the information system and other technologies in pharmacy.

Qualifications:
Education
Required: Bachelor of Science in Pharmacy or Doctorate in Pharmacy
Preferred: Residency
Work Experience
Required: Completion of Internships and Externships as outlined by Board of Pharmacy.
Preferred: Hospital pharmacy experience including IV admixture and Oncology.
Licensure/Certifications/Registrations
Required: New Hampshire Board of Pharmacy licensed Registered Pharmacist or registration eligible.
Preferred: BCPS
Other Skills/Knowledge
Required: Should be detail-oriented, able to work in emergency situations; able to prioritize tasks, have effective verbal communication and interpersonal skills, and a high level of computer literacy.
Preferred: Experience using the Cerner Millennial Information System
