QueensCare is a 501 (c)(3) nonprofit organization originally founded by the Franciscan Sisters of Sacred Heart, whose mission of compassion and accessible healthcare continues today. QueensCare provides screening, treatment, education, and preventive and direct care to Los Angeles area residents in need. Beyond that, we also give charitable grants to other non-profits making a difference in the City of Angels and fund scholarships for those entering medical fields with an interest in working in underserved communities.
About the role
The Community Health Nurse (CHN), supervised by the Community Health Manager (CHM), delivers direct and accessible patient care to underserved communities across Greater Los Angeles. Operating in diverse settings like places of worship, food banks, schools, and shelters, the CHN serves as an educator, coordinator, health case manager, evaluator, and facilitator of health services. The CHN focuses on promoting wellness and fostering positive health behaviors among individuals and groups.
What you"ll do
Community Nurse Engagement, Support, and Responsibilities
- Conducts preventative health screenings for diseases and conditions such as obesity, diabetes, hypertension, and high cholesterol.
- Conducts health assessments to identify risk factors for chronic conditions like cardiovascular disease.
- Practices wound care techniques for injuries.
- Performs follow-up calls to assist patients with Primary Care visits, medical procedures, medication adherence, and other medical concerns.
- Tracks patient progress, adjusts care plans, and conducts clinical follow-up to support patient self-care goals.
- Administers vaccines and provides health education, health counseling, and intervention plans at events and assigned sites.
- Provides health information, resources, and referrals based on Adverse Childhood Experiences (ACEs) scores.
- Acts as a clinical advocate to overcome barriers to health promotion and disease prevention.
- Generates referrals to Community Health Workers for social services and care continuity.
- Completes proficiency testing for Nondiagnostic General Health Assessment (NGHA) screenings during API testing periods to meet Los Angeles County NGHA guidelines while delivering patient care.
- Participates in Health Cabinet Meetings to identify healthcare gaps and enhance community health initiatives.
- Maintains productivity standards and ensures accurate and timely documentation in Electronic Health Records (EHR).
- Provides crisis intervention and follows reporting protocols as needed.
- Follows CDC infection control guidelines, sanitizes equipment, monitors and records refrigerator temperature logs, and maintains inventory and vaccine transportation in compliance with LA County guidelines.
- Works together with the Mental Health Program, Mobile Programs, and Pastoral Care Department to establish connections and facilitate access to services.
- Provides mentorship for colleagues, students, volunteers, social service agencies, and health ministries.
- Collaborate with other programs and community organizations to provide links to services and enhance service delivery.
- Travels daily or frequently to various partner sites within Los Angeles County.
- Perform other duties as assigned.

Qualifications
EXPERIENCE
- Experience working with adults, children, and families from diverse social, cultural, educational, and economic backgrounds.
- A minimum of two years of experience working in clinical and acute settings is required; community health experience is preferred.
- Experience working with faith communities and demonstrating spiritual maturity is preferred.
- Required active and unrestricted Registered Nurse (RN) license in the State of California is required.
KNOWLEDGE
- Demonstrates proficiency in computer applications such as Zoom, Teams, web-based portals, Microsoft Excel, Power Point, Word, Outlook, MyTurn, EMR, and CAIR.
- Promotion of process improvement.

The pay range for this role is:
56.10 - 56.10 USD per hour(QueensCare Corporate Headquarters)
