Job Summary: This position is under the direction and supervision, as assigned, of the Clinic Supervisor, Social Work Supervisor, Behavioral Health Manager or Home Care CNA Supervisor. Obtains information from participants as directed by providers and/or supervisor in regards to referred services. Participant Care Coordinators will also obtain information from participants and from their respective team meetings for overall planning and coordinating of services. Does scheduling for participant appointments, including outsides consultations, inpatient/outpatient, residential programs and diagnostic studies. Schedules for CNA and/or registry staffing for their assigned departments as needed. Ensures referred service providers have pertinent information to treat or provide services to PACE participants, notifies participants of upcoming appointments and schedules transportation when needed.
Qualifications and Requirements:
Education: High School Diploma or equivalent required
Experience: Two years of administrative support experience.
Skills and Knowledge:
- Knowledge of organizational policies, procedures and systems.
- Knowledge of computer systems and word processing.
- Knowledge of grammar, spelling and punctuation with excellent verbal and written communication skills.
- Knowledge and ability to perform multiple and complex clerical task with computers.
- Skilled in taking and transcribing dictation and in the operation of office equipment.
- Ability to communicate and present information.
- Ability to identify problems and recommend solutions.
- Ability to establish priorities and coordinate work activities.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Skilled in establishing and maintaining effective working relationships with patients, co-workers, medical staff, and the public.
- Skilled in identifying and recommending problem resolution.
- Ability to work independently with minimum supervision.
- Ability to communicate clearly and effectively.
Physical Demands and Working Conditions:
Physical Requirements - Requires manual and finger dexterity and eye-hand coordination; the ability to use department equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics.
Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, vendors, and clients.
Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded and fluctuating indoor temperatures. Moderate pressure to meet scheduled appointments while dealing with frail and confused participants. Subject to people that may have the potential for verbal or physical aggression.
Environmental Conditions – May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a clinic environment . And the possibility of exposure to areas of infestation. Subject to unpleasant odors.
Duties and Responsibilities:
- Maintains daily schedules for assigned discipline.
- Schedules for CNA and/or registry staffing for their assigned departments as needed.
- Performs any necessary documentation in medical records, and performs computer entries to update participant information.
- Schedules necessary appointments of participants for clinic, specialists, outside medical appointments and home care services when requested. This may include scheduling across multiple sites.
- Coordinates follow-up appointments, ensures service provider has pertinent information to successfully care for the participant in a timely manner.
- Completes miscellaneous projects at direction of their assigned supervisor as assigned.
- Maintains and updates files, binders, records, emails, and inventory lists
- Consults each day with their team on managing the participant flow for the day. Assists in prioritizing the scheduling of participants.
- Maintains exam rooms for necessary supplies and materials. Ensures cleanliness and that exam rooms are properly stocked. Prepares list of medical supplies needed.
- Directs, screens, and handles telephone calls.
- Helps in prioritizing work activities, evaluating effectiveness, and modifying activities as necessary.
- Identifies, analyses, and resolves work problems.
- Ensures that requests for information are handled promptly and effectively.
- Follows all SPP Standard Operating Procedures, OSHA safety guidelines, and appropriate infection control procedures.
- Maintains the confidentiality of all company procedures, results, and information about patients and clients including faxed information and courier materials.
- Maintains safe working environment. Follows all fire and safety guidelines.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Performs other duties as required or requested.
