Job Essentials
On an as needed basis, this Pediatric Dentist will provide services in to patients of Primary Children's Hospital network. This position is responsible for communicating effectively when speaking or writing. This individual must demonstrate ethical conduct in all activities related to dental practice, and is responsible to understand and follow all requirements of legal, regulatory, and accrediting agencies affecting operations.
Minimum Qualifications
+ Dentist must obtain (i) an unrestricted license to practice dentistry with a minimum Anesthesia Class B Permit in Utah; (ii) an unrestricted controlled substance license in Utah; and (iii) an unrestricted federal DEA certificate for schedules II–V.
+ Dentist must complete residency in either a CODA (Commission on Dental Accreditation) accredited Pediatric Dental or Orthodontic residency.
+ Dentist must be board certified or board eligible for ABPD (American Board of Pediatric Dentistry) or ABO (American Board of Orthodontics).
+ A query of the National Practitioner Data Bank must show no adverse actions.
+ Dentist must be in good standing at all current hospital, medical staff, and other affiliations.
+ Dentist must be in good standing with SelectHealth or be qualified to become a SelectHealth participating provider.
+ Basic Life Support Certification (BLS) for healthcare providers.
+ Pediatric Advanced Life Support Certification (PALS).

$7.25 - $999.99
