About the Job
Community Health Network, in collaboration with Community Hospital Oncology Physicians, LLC, is seeking an experienced Nurse Practitioner (5 years experience minimum) to collaborate with medical/hematology oncologists.

Desired requirements
· Minimum of five (5) years’ experience in a clinical setting (beyond training)
· Board certification and IN medical state license required
· Desire to function in a highly collaborative atmosphere
· Consistent and consecutive employment and training history
· Excellent communication skills fostering collaboration with all members of care team
· Develop and maintain relationships with referring physicians
