WHO WE ARE LOOKING FOR:

Specialty Physicians of Illinois, LLC, is seeking an experienced Family Medicine/Geriatric Nurse Practitioner to work full-time & join our team at Franciscan Health Olympia Fields Medical Pavilion in Olympia Fields, IL to help meet the growing demands of our outpatient program. Provider will monitor and care for our patients going and/or returning to nursing homes or skilled nursing facilities.

WHAT WE NEED:

Ideal candidate will have knowledge of discharges, nursing home care, and experience with family medicine and/or geriatric care. Candidate would thrive in a fast-paced, autonomous, self-managed environment. Provider will need to work collaboratively to coordinate and deliver the highest quality of patient care. Candidate will also have desire to develop strong ties with patients and the community in which they serve.

WHAT YOU CAN EXPECT:

• Flexible full-time schedule; no weekends or holidays
• Average inpatient and outpatient volume: varies
• Provider will take own call until 8pm, Monday - Friday
• Daily Duties:
  • Day-to-day care of patients
  • Review medical histories and medical assessments
  • Implement plan of care with patient and family
• Will work with skilled and long-term patients
• EPIC electronic medical record

Qualifications:

• Master's Degree - Required
• Nurse Practitioner (NP) or Nationally Recognized Certification - Required
• Basic Life Support Program (BLS) - American Heart Association - Required
