Description
SUMMARY
Under the supervision of the RN Supervisor and/or Program Director, provides support to patients and documentation of self-medication. This position is Per Diem, and schedules vary.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and responsibilities may be assigned.
+ On Site daily
+ Support residents in the (self) administration of medication
+ Documentation of (self) administration of medication
+ Performs TB and Drug screenings on all patients within 72 hours of admissions
+ Performs routine urine drug screens as directed by the treatment/recovery plan
+ Monitors and records vital signs
+ Provides triage and provides medical services accordingly
+ For patients with health care needs, schedules, and coordinates all medical appointments, and provides periodic follow-up
+ Reviews and transcribes medication orders and monitors medication records for compliance.
+ Observes and records response to treatment/medication and notifies the Medical Director, the RN Supervisor, and the Program Director of any such instances
+ Monitors ongoing/chronic medical problems
+ Handles minor needs of patients including attending to minor injuries, administering routine medications
+ Maintains all medical records
+ Performs QA activities
+ EKG
+ Performs accurate and complete audits of medication transported, received, administered, stored, or disposed, as trained, and assigned, in accordance with Phoenix House medication policies and procedures.

Qualifications
REQUIRED EDUCATION
+ Graduate from an accredited nursing program
LICENSES/CERTIFICATIONS
+ Current NYS license
+ Current certification in CPR, Child Abuse, and Infection Control
REQUIRED EXPERIENCE
+ At least three years related experience
+ Qualified by training, education, and experience in providing nursing care for chemically addicted patients and patients withdrawing from alcohol and or drugs
WORK ENVIRONMENT
+ This job operates in an indoor office environment. This role routinely uses standard office equipment such as computers, phones, and photocopiers.
