The Palmettos of Parklane - Memory Care Manager
This is a full-time position and reports to the Director of Health Services and Executive Director
The Program Director plans, organizes, develops and leads the overall operation in the memory care unit in accordance with federal, state and local laws; partners with the Activities Director to ensure a variety of appropriate activities are available throughout the day and evening and that Care Managers are actively involved and engaged with activities; designs, schedules and facilitates the memory care unit program incorporating Life Skills and a variety of dementia appropriate activities; establishes a cooperative relationship with the local Alzheimer s Association chapter or other Alzheimer s advisory organization; ensures monthly family support group meetings are planned and facilitated; supervises C.N.A.s in the memory care unit.

SUPERVISORY RESPONSIBILITIES
Responsible for the direct supervision of care giving and coordination of health care needs within the residence.

Job duties to include but not limited to the following:
- Implement and coordinate dementia program.
- Provide support and training to family and staff.
- Coordinate staff schedule accordingly.
- Communicate effectively with the medical/health care community, patients and families.
- Interpret and implement regulations (state and federal).
- Assess each individual resident to determine interests and needs.
- Provide therapeutic activities which meet the individual interests and needs of each patient.
- Assist residents with activities of daily living according to the individual service plan.
- Assist residents with medication as defined in the medication procedure.
- Facilitate continuity of care for those residents receiving home health services.
- Provide supervision and monitoring of staff in the supervision of medications, to include frequently auditing medication records.
- Organize or assist the residence director of health services in organizing, developing, and conducting service plan reviews as required with appropriate resident care team members and resident families.
- Assign weekly blood pressure and weights to rotating shifts.
- Assure in-house ancillary medical services such as podiatrist, doctor visits, dental visits, psyche visits, lab, X-ray, ambulance, etc. are scheduled and followed through.
- Assure that family members are aware of resident's need for scheduled physical, dental, or eye appointments.
- Perform daily and monthly safety checks of the facility and grounds, assisting the director of health services and executive director for resident safety purposes.
- Participate in departmental quality improvement activities.
- Participate in residence functions, including family and social functions.
- Shares on-call responsibility per rotating schedule as required
- Working at a minimum one Saturday a month (to be comped by a week day) as well as unscheduled 3 rd shift site visit at least once a month.
