Our client is a 25-bed, non-profit community hospital designated as a Critical Access Hospital. WHO YOU ARE:
Under the general supervision of the Practice Director, is responsible for assisting the Physicians, Advance Practice Professionals, and practice team with direct and support related patient care, while treating patients in a respectful, confidential, and friendly manner.
Responsibilities
Clinical
- Documents patient's chief complaint and medical history, and completes pertinent forms in accordance with best practice workflows. Prepares Provider for a successful appointment by communicating pertinent information gleaned from pre-visit/chart prep completion
- Performs phlebotomy for needed testing
- Performs EKGs, rapid strep screen, flu swab, INR, pregnancy tests and other tests that may be ordered by the Provider in accordance with CLIA waived regulations
- Assists in minor procedures such as pap smears, suturing, dressing changes, catheterizations and casting. Practices aseptic and sterile techniques as needed
a. Prepares and administers medications as directed by a Provider, utilizing best practice medication processes such as the 5 R's of right med, right dose, right route, right patient, right time.
b. Provide the patient with educational materials as requested by the Provider. Instruct patients about medications, diet, exercise, and follow up instructions - Assists the Practice Coordinator with compliance for Prescription Drug Monitoring Program which includes communicating with patient regarding the program requirements; knowledge of the contract contents, etc
- Assists with robust check out of patients after appointment completion to understand potential barriers for care plan follow through such as appointment transport issues; difficulty with medication procurement due to financial constraints; recommended test completion, etc
- Works with the Provider to suggest assistance from Care Coordination for times when patient reveals complex barriers to health maintenance
- Is able to conduct a nurse visit (EMR term) at the request of a Provider for such needs as hypertension screening; hypertension rechecks after medication change; INRs; diabetes follow-ups; etc., documenting appropriately
Patient Access
- Accepts and returns patient calls under the supervision of the nurse or Provider, using appropriate telephone etiquette
- Schedules appointments and processes orders for patients for appointments, procedures, tests, referrals, and/or consultations as requested by the Provider
Clinical Support
- Performs pre-visit planning/chart preloading in accordance with established guidelines, including but not limited to, obtaining appropriate records from specialists, emergency department visits, hospital stays, and primary care to ensure an efficient visit for the patient and the Provider
- Prepares the examination room with needed supplies, instruments, and equipment and maintains room in a clean, professional, and welcoming manner
- Greets patients in a professional, friendly manner, escorting them to exam rooms while introducing themselves and explaining the rooming process
- Prepares/instructs/positions and drapes the patient
- Ensures the Provider is on schedule being mindful of emergencies and unexpected interruptions
- Prepares specimens with documentation for processing in accordance with Laboratory regulations
- Cares, cleans, and sterilizes equipment per instruction or established procedures
- Orders medical supplies and medication/immunizations as needed, and keeps exam rooms and bathrooms stocked
- Disposes of used supplies properly and follows all established infection control and biohazard policies and procedures
- Calls orders to pharmacies for medications utilizing the approved prescription refill policy and procedure under the direction and supervision of the Provider
- Ensures vaccines are handled and administered in accordance with standards of the NH Immunization Program with provision for reporting of adverse events
- Completes and processes referrals, pre-certifications or pre-approvals as needed
EMR Utilization
- Utilizes the current Electronic Medical Record to complete the documented Best Practice Workflows accurately
- Documents and updates the patient's medical history within the EMR guidelines, including: vital signs, past medical history, social history, surgical history, family medical history
- Completes the daily work list (order tracking, tasking, document import) providing timely care
MINIMUM EDUCATION AND WORK EXPERIENCE
- One to two years' experience in the medical field unless successful completion of an accredited Medical Assistant Program
- Current CPR
