Verrazano Nursing and Post-Acute Center is looking for an MDS Coordinator to join our team. We are seeking qualified candidates who have experience as an RN and are committed to help our patients and facilities receive the support they need.

MDS Coordinator schedules and completes resident assessments in compliance with State and Federal Guidelines in order to meet requirements of Medicare and Medicaid.

Responsibilties: 

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

Job Type: Full-time

Pay: Negotiable

Requirements:

• Must have prior MDS Experience
• Valid NY RN License
• SHort Term/Long Term Care Experience Required
• Must be organized, and professional
