MDS Coordinator (RN) - Alliance Health at Marie Esther

POSITION PURPOSE:

Coordinates the Minimum Data Set and care planning activities for the facility.

EDUCATIONAL REQUIREMENTS:

Graduation from an accredited RN program.  Bachelor's degree preferred.

EXPERIENCE REQUIRED:

One to two years of nursing experience required.  One year of long-term or sub-acute care experience preferred.

PROFESSIONAL LICENSURE AND CERTIFICATION REQUIRED:  Current Massachusetts RN licensure.  Responsible for re-licensure per federal and state regulations.  Compliance with yearly CEU requirements.  Current CPR, Accucheck, and IV Therapy certification.

INTERPERSONAL RELATIONSHIPS:

Communicates effectively with residents, families, visitors, co-workers, physicians, and other employees.

PHYSICAL REQUIREMENTS:

Physical health sufficient to meet the ergonomic standards and demands of the position and its location.  Regular and consistent attendance at the facility.

SUPERVISORY RESPONSIBILITIES:

None

REPORTING RELATIONSHIPS:

Reports to the Director of Nursing.
