WCS Healthcare has an immediate opening for an RN Case Manager with a large Social Services Agency with needs in Suffolk County.
Job Function:
- Day Shift
- 8:30am-5pm flexible on start time
- Assess individual's needs.
- Build strong relationships with individuals, families, and the treatment team.
- Create and ensure support staff stick to nursing care plans.
- Schedule appointments for individuals promptly.
- Teach individuals and support staff regarding nursing plans of care required.
- Document individuals status, health and follow-ups.
- Monitor health records such as seizure charts, bowel movement tracking, vitals, etc.
- Manage medication storage properly, dispose of expired medications correctly, and keep the medication area clean.
Job Requirements:
- Registered Professional Nurse with licensure from New York State Education Department
- 1 year nursing experience working with Intellectual Disabled Adults or Case Management experience preferred
