We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our home health team. Always Best Care has become one of the area’s leading providers of in-home care, and senior living referral services. The SLP provides professional services within the scope of Speech Pathologist practice standards in collaboration with primary care physicians, teaches, and educates patients and their families. We are expanding our top-notch services to provide licensed medical care to our beloved patients in the home helping to take the burden off our families’ shoulders.
Join our team as we expand our in-home medically licensed services for our patients. We are seeking per diem employees as our new home health division launches. As a per diem employee, you will have the flexibility to manage your schedule and provide specialized care. As the home health division expands, full-time salaried employment will be an option in the future. Compensation
Per diem – Specific visit based rates to be finalized at interview.
$32 - $45/hr based on experience

Physical Requirements/Special Equipment
Must be able to access Patient’s homes that may not be handicap accessible.
Walking, standing, sitting, reaching, bending, climbing stairs, possible transferring/lifting devices, lifting/transferring 150 pounds or greater daily.
Possess a registered and insured vehicle to perform routine patient visits.
Able to hear, speak, understand and communicate effectively in English.

Responsibilities
- Receives and prepares the intake referral information
- Obtains patient’s medical history from the patient, family, or significant other
- Evaluates the patient’s speech and language abilities and perform periodic re-evaluation
- Plans and provides rehabilitative services related to speech and language disorders
- Prepares and records type of treatments, and patient’s response on clinical progress day of visit and within 7 days in patient’s medical record
- Assists physician in evaluating the level of function
- Develops an individualized ST Plan of Care (POC) to be submitted to the physician for approval and implementation and performs necessary revisions based on patient needs
- Functionally reassess the patient for ongoing therapy
- Determines medical necessity for other services
- Selects and administers diagnostic and therapeutic ST techniques and materials
- Effectively communicates ongoing with patient and family progression/changes in POC
- Effectively communicates with other disciplines in the case (case conferencing) and establish best practices
- Effectively communicates with the Director of Patient Care Services/Nursing Supervisor regarding scheduled visits with the patient’s MD to obtain effective treatment modalities and/or rehabilitative therapy modalities
- Submits requests for re-authorization for “more visits needed” to payers on a timely basis
- Coordinates community services for the patient to assist in safe home care needs
- Perform and submit timely clinical record reviews (CRR)
- Participates in the planning, operation, and evaluation of the therapy services for the organization
- Other duties as assigned
Qualifications
- Master’s degree in Speech Pathology
- Active California license “in good standing”
- Minimum 1-year of experience as an ST, preferably in a home-health setting.
- CPR certified
- Valid California driver’s license and proof of insurance (Military exception)
- WellSky/Kinnser experience is a plus
- Complete a skills test
- Problem solver
- Detail oriented
