We're seeking a dedicated Social Worker to join our team and play a vital role in supporting our residents' social and emotional well-being.  In this position, you'll work directly with residents and their families, collaborating with the broader care team to ensure a holistic approach.

Making a Difference for Residents:

Conduct resident and family interviews to gather social history and build strong relationships.

Participate in resident assessments and discharge planning to ensure a smooth transition.

Develop and implement individualized social care plans that address residents' unique needs.

Advocate for residents by connecting them with appropriate community resources and financial assistance programs.

Interpret residents' social, psychological, and emotional needs to the care team, fostering a comprehensive understanding.

Collaboration and Support:

Provide ongoing support and guidance to residents and their families.

Consult with staff and community agencies on best practices for meeting residents' social and emotional needs.

Assist in obtaining resources from community agencies to effectively address resident needs.

Uphold the highest standards of confidentiality for all resident information.

Additional Responsibilities:

Assist in developing and implementing department policies and procedures.

Review department policies and procedures regularly and recommend necessary revisions.

Complete required documentation, forms, and reports in a timely manner.

Participate in ongoing professional development opportunities.

Who We're Looking For:

Bachelor's degree in Social Work (MSW preferred).

Strong interpersonal and communication skills to build rapport with residents, families, and staff.

Excellent assessment and planning skills to develop effective social care plans.

Ability to work independently and as part of a team.

Commitment to social justice and a passion for improving the lives of others.
