The Children's Medical Center, PA is a private, multi-site Pediatric practice located in the Upstate of South Carolina. We are a Patient-Centered Medical Home (PCMH) and are committed to improving the health of our population and communities. Along with our team of Pediatricians, Physician Assistants, and Nurse Practitioners we offer lactation and mental health services to support our providers and communities.
The Outpatient Therapist provides family and individual therapy for patients, and they are an integral part of a multi-disciplinary team that includes the patient's medical providers. The integrated model provides streamline support for the whole patient through onsite therapists embedded within each site. Patient population ranges from aged 4-25 years of age. Supervision is provided to LPC-A's on an ongoing basis through onsite and day to day support.
Responsibilities
- Asses the patients medical history, mental health history, and current presentation to understand the patients needs.
- Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in areas of responsibility.
- Treat various mental, emotional, and behavioral disorders.
- Performs social work consultation for patients and caregivers.
- Conducts psychosocial assessments.
- Provides supportive, crisis bereavement and other social work interventions as necessary.
- Develops a plan of care either individually or with input from the patient, family, physicians, and other members of their healthcare team.
- Modifies care plans to reflect changes in patient status.
- Refers patients to other services to improve their well-being.
- Educates, communicates, and coordinates with in-house Pediatrician and outside agencies as necessary to ensure the patient receives quality care and to identify and resolve delays and issues.
- Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multi-disciplinary care team, outside agencies and other resources as necessary.
- Completes timely and accurate documentation in the medical record using knowledge of documentation standards for the department to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing standards.
- Complies with federal, state, local, and clinical program/facility standards.
- Acts within the scope of the Mental Health Practice Act.
- May provide psychotherapeutic modalities appropriate to the level of care.
- May ensure that productivity standards and expectations are met.
Qualifications
- Currently holds an LPC-A certification in South Carolina
- Master's degree in professional counseling or related field from an accredited institution.
- Experience working within a healthcare setting and desire to work with pediatric population
