Job Title: LCSW/LICSW

Pay: 33-40/hr DOE

Overview

• Clinic to start off day with provider huddles to discuss patients
• Will schedule their own appointments with patients
• Assessments in home for fall risks, etc.
• Documentation in EMR system
• Interactive with Nurse Case managers, providers and the rest of the care team

Caseload:

• LICSW is utilized as needed with patients (providers will put in an order for the LICSW to meet with them)
• Up to 6 patients a day including telehealth visits
• Candidates must be comfortable with group therapy about once a week

Patient Population:

• Elderly population (Medicare)
• Depression, substance abuse

3-5 Must Have Skills/Qualifications

• LICSW in Alabama
• Understanding of local resources
• Experience working in a clinical setting is highly preferred
• Experience working with patients who have substance abuse issues is highly preferred

Nice to Have Skills

• Experience Working with Seniors and Medicare population
• Ability to speak strongly and confidently when making care recommendations for patients to providers and rest of the care team.
• Group therapy experience

Schedule/Shift

• M-F, 8am-5pm
• In person at the Montgomery office
• Only one office in this region
• Visits can be in the office in person, virtually, or in the field
• Social worker will be in the field visiting patients in the home approximately 25% of the time
• They will be able to set up their schedule
• Typically see an average of 6 patients per day (this includes in office, field visits, and telephonic visits)

The LCSW will be an asset to the Care team as an expert in behavioral health space. They will be able to help shape the social work department and support patients requiring mental/behavioral therapy

• Will do home visits with patients
• Home assessments for fall/injury risk
• Field based - car allowance and mileage reimbursement
• Hold Therapy sessions to those prescribed by provider in office or telephonic
• Group therapy as needed
