Position Overview:
The Lead Care Manager LCSW plays a crucial role in coordinating and managing the care of complex patients, ensuring a smooth transition of care across the continuum. This position requires strong clinical judgment, excellent communication and interpersonal skills, and the ability to work effectively as part of a multidisciplinary team.
Key Responsibilities:
- Care Coordination:
- Develop and implement comprehensive care plans for complex patients, including those with chronic illnesses, multiple comorbidities, and social determinants of health.
- Coordinate care with physicians, nurses, social workers, and other members of the healthcare team.
- Facilitate communication and information sharing among all providers involved in the patient's care.
- Discharge Planning:
- Develop and implement discharge plans that address the patient's medical, social, and emotional needs.
- Coordinate post-discharge services, such as home health care, skilled nursing facility placement, and community-based support services.
- Case Management:
- Monitor patient progress and address any barriers to care.
- Advocate for patients' needs and ensure access to necessary resources.
- Conduct care conferences and participate in interdisciplinary rounds.
- Team Leadership:
- Mentor and supervise junior care managers.
- Assist in the development and implementation of care management protocols and best practices.
- Participate in quality improvement initiatives and performance improvement activities.
Qualifications:
- Education:
- Master's Degree in Social Work (MSW) from an accredited program.
- Licensure/Certification:
- California Clinical Social Worker (LCSW) License required upon hire.
- Experience:
- Minimum of two (2) years of experience in a healthcare-related field (Acute, Ambulatory, or Post-Acute).
- Minimum of two (2) years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
