Summary:
The LPN/ Program Manager directs and manages the operations and activities of the home to comply with all rules, regulations, and policies that apply. To provide a needed service by adopting a person-centered approach that fosters self-awareness, personal growth and gives each person the strongest voice with regards to decision making and lifestyle choices.
Essential Duties and Responsibilities:
The LPN/ Program Manager directs and manages the operations and activities of the home to comply with all rules, regulations, and policies that apply. Essential Duties and Responsibilities:
- Core duties and responsibilities include the following. Other duties may be assigned.
- Follow up with all parties involved regarding clients’ care i.e. phone, care conferences, and email.
- Manage resident’s schedules and weekly appointments.
- Supervise in-home staff.
- Train and/or direct appropriate person in the training of staff for on-site and areas such as challenging behavior, aging population support/personal care.
- Ensure staff attend all mandatory and refresher training.
- Coordinate training for staff to ensure their certifications are current.
- Adhere to all state and federal regulations and agency policies and procedures.
- Participate in required and meetings required by the supervisor.
- Make sure the assigned duties of staff are completed.
- Lead team meetings with staff and clients.
- Where necessary ensure that all disciplinary and grievance procedures are carried out in line with agency policy to ensure timeliness, accuracy, thoroughness, and compliance with regulations and policies.
- Recognize the signs and symptoms of client abuse and neglect, misappropriation of funds, and report to appreciate person(s).
- To ensure services provided respect the resident as an adult and support their individuality and uniqueness in all areas of life.
- Implementing/monitoring and contributing as required, Person-Centered Plans.
- Responsible for the fiscal management of the home and oversee resident funds.
- Ensure the home is kept clean and hygienic throughout and meets state and federal regulations.
- Manage staff schedule.
- Provide regular on-call cover.
- Perform direct care.
- All other duties as assigned by the Program Director.
Supervisory Responsibilities:
To lead by example, in a positive working environment and develop effective teams within the Home. Provide regular supervision and annual appraisal to staff in line with the agency’s procedures and standards. To ensure good quality working relationships are built and maintained between staff and the individuals, they are supporting.

Education/ Experience:
- Equivalent combination of education, training, and experience that would provide the required knowledge and abilities. LPN License in the state of MN.
- One year of supervisory experience in personal care services or in a related program.
