Responsibilities
Summit Medical Group's Corporate Office is seeking a LPN - Quality Outreach Coordinator to facilitate provider and member outreach to ensure member receipt of important clinical services to improve member health outcomes. This is a full time opportunity, working Monday-Friday 8:00AM to 5:00PM.
** Current License in Practical Nursing required **
Responsibilities: (List does not include all duties assigned)
- Makes outbound calls to members to provide assistance in obtaining services (i.e.,provide reminders, make appointments, arrange transportation, etc.) to avoiddelays in treatment and improve member health outcomes.
- Distributes educational information to members in accordance with the HEDISoutreach work plan
- Records and documents all member outreach activity.
- Perform chart review as necessary to verify care gap status. If care gaps are open,will follow workflow for outreach.
- Support practices and patients by scheduling appointments based on set protocolsfor MAWV and/or overdue screenings.
- Follows call scripts in accordance with the quality outreach work plan.
- Provide administrative support to HEDIS activities to achieve quality goals,including but not limited to: retrieving medical records, remote and/or onsiteprovider office data collection, formulate and compile monthly reports.
- Place orders per protocol to support HEDIS gap closure.
- Performs related work as assigned
Qualifications
Education: High School diploma is required. Associate degree preferred.
Experience: Prefer a minimum of one year of medical experience in healthcare quality.
Certification/License: Current License in Practical Nursing required.
