Job Location: Midtown Manhattan, NY; 4 days in office, 1 day remote, with occasional travel to clinical sites in NY & NJ areas.
Job Schedule: Mon-Fri, Day Shift (9am-5pm)
Job Type: Full Time
Pay Rate: $60,000-$70,000 per annum ($30- $33/hr)

Position Summary:
VitalCheck Wellness is seeking a Licensed Practical Nurse (LPN) to join our team within our corporate office to provide coordination for our specialty programs.
You will be responsible for supporting employees of our clients in the NY and NJ area to ensure patient adherence and provide education with minimal clinical duties.
We are looking for someone passionate about improving patient's lives and delivering high level customer service.
Responsibilities:
- Coordinate between patient, clinical and operations partners to set up appointments, follow up, and get feedback.
- Make sure there is high adherence rate to the program in accordance with our guidelines.
- Provide education around diabetes management and exercise advice to help clients establish healthy eating habits and lifestyle.
- Support clients in diabetes management and other aspects of well-being and lifestyle.
- Establish and maintain rapport and good relationship with patients.
- Maintain clients documentation.
Requirements:
- Bachelor's degree in nursing, health administration, or a related field.
- Active license of Licensed Practical Nurse (LPN) in NY or NJ strongly preferred.
- Three to five years of experience in nursing, health administration, or a related field.
- Experience with educating diabetes or other chronic conditions.
- Experience with vaccine and phlebotomy strongly preferred.
- Proficient with computers, Microsoft Office, EMR systems.
- Customer service-oriented and have great bedside manner.
- Strong communication skills; both written and verbal.
- Ability to work independently with little supervision.
- Bilingual with Haitian Creole or Spanish speaking strongly preferred.
- Must be able to reliably commute throughout NYC and/or NJ.
