About the Job
Nurse Case Manager
Coordinate the delivery of health care services for adults with disabilities residing in residential programs.
Oversee general health issues for people with autism in Residential programs or day programs
Monitor medication and medical records for people receiving services
Consult with MD's to ensure optimal medical treatments are provided to people being supported
Supervise direct support professional (DSP) staff administration of medication to people receiving services
Ensures medical follow up (i.e. appt.'s, labs obtain prescribed medication, etc.) occurs in a timely manner
Ensures individuals are receiving prescribed medications, treatments and nutrition/diet as ordered
Accompanies/transports individuals to appointments when needed, due to staffing needs
Maintains accurate mandated medical records
Attends internal team meetings as scheduled
Qualifications & Work Experience
1-2 years LPN experience highly preferred
Valid NYS LPN license in good standing required
Associates Degree in Nursing required
Knowledge of OPWDD regulations is highly preferred
BLS, CPR certified; SCIP-R certification required
Ability to safely assist lifting individuals of various weights & 20 lb. items required
Organizational skills, and Interpersonal skills necessary to interact effectively with coworkers, individuals served, families and outside contacts is essential
Must be able to maintain Individual/family confidentiality
+ Be responsible for performing nursing assessments and diagnoses
+ Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.
+ Provides guidance and training to direct care staff to ensure proper delivery of health care services.
Requirements include:
+ NYS RN License
+ Prior nursing experience and/or experience working with persons with developmental disabilities preferred.
+ Excellent oral and written communication skills.
+ Ability to travel to and access multiple locations throughout the metropolitan area.
+ Able to work flexible hours and/or days.
