Unit/Department Assisted Living
- # Locations: 10
- Type of Facility Assisted Living: Memory Care, A few Indendent living. All memory care are secured
locked units. Free standing facility.
- # of Beds/Residents : See below-- depends on location.- majority are private room
- EMR: Point Click Care - assessment and nurse notes, only. Medication is not tracked electronically
- Ratios: "RN's: 1:40 on Day shift ; RN is responsible for overseeing the nurse aides on shift, as well
LPN's: 1:40 on days evening nights
CNA's:days 5, evening 4 nights 2-3"
- Required Certifications Licensure,: BLS; CNA are not required to be certified, but it's preferred
- Common diagnosis / Types of patients: Predominantly memory care, dementia, Alzheimer's
- Job Duties:: State Dependent by regulation. Rarely have IV, Enteral, Ostomy, Trach, Would not
have vents in Assisted living.
- Experience or 'Must have' skills: Dementia education requirement; There are times when there isn't
an RN onsite. Per regulations this is acceptable, however, an RN has to be on-call at all times.
- Are there IV's or titratable drips?: No; not required. Managed by home care or hospice if needed.
- Are RN's required to attend team conferences for acute inpatient rehab? N/A its Assisted Living
- DON/Charge Nurse/House Supervisor: RN working on shift is charge nurse
- Physician availability after hours: Yes, via phone, or telehealth in some markets
- PT/OT/SLP, Pulmonary or Cardiac Rehab, Case Managers, Social Workers, Dieticians? ": All of these disciplines would be
by contract and dependent on location "Is RT on-site 24/7
- Oxygen Therapy /Equipment (ie Cpap, Bipap, High Flow) ": No, Rare that CPAP, BIPAP are used in Assisted Living. High
Flow Oxygen would not be.
- Pharmacy on-site? Medication dispensing system?: Pharmacy 24/7
- Shifts & Scheduling: "8 hour shifts | Day 7a-3p; Evening 3p-11p; Nights 11p-7a
CNA's: 4-5 day shift; 3-4 evening shift; 1-2 night shift.: No Self Scheduling, I nurse per shift RN or
LPN- Day
shift has additonal support of RSC (DON) RN preferred. Staffing dependent on census- CNA Days
5 Evenings 3-4 Nights 2-3"
- Weekend Requirement: Every other weekend
- Holiday Requirement: Rotation
- Call or Standby Requirement: Resident Servise Coordinator 24/7 ( passes meds, CNA duties)
- Floating Requirements : Yes, could float to near by sister location (including SNF)
- Scrub Color/Dress Code: no color requirement
- Unit/Department Specific Orientation: 3 days of floor orientation, including electronic orientation.
