Job Overview - Intake and Outreach Manager (RN):
Take charge as the Intake and Outreach Manager (RN) with our client in Bronx, NY, overseeing the intake process for the Certified Home Health Agency (CHHA) program. In this full-time role, you'll streamline admissions, coordinate with the nurse care manager, and collaborate with partners to optimize patient care and referral networks.
Compensation: $105,000 - $110,000/year
Location: Bronx, NY
Schedule: Monday to Friday, 9:00 AM to 5:00 PM
Responsibilities as the Intake and Outreach Manager (RN):
Take charge as the Intake and Outreach Manager (RN) with our client in Bronx, NY, overseeing the intake process for the Certified Home Health Agency (CHHA) program. In this full-time role, you'll streamline admissions, coordinate with the nurse care manager, and collaborate with partners to optimize patient care and referral networks.

Responsibilities as the Intake and Outreach Manager (RN):
- Admission Management: Oversee the screening, evaluation, and admission planning processes for the CHHA program, from inquiry to final acceptance.
- Coordination: Work with the Nurse Care Manager and other internal and external vendors to ensure timely patient assessments and admissions.
- Development: Partner with the Associate Administrator of Provider Relations to establish new referral sources and strengthen existing partnerships.
- Direct Care: Occasionally perform the duties of a Nursing Care Manager as needed, including patient assessments and care planning.
- Licensure: Active Registered Nurse (RN) license and registration in New York State required.
- Education: Bachelor of Science in Nursing (BSN) required.
- Experience: 2+ years in patient assessment and care planning for home health, including 1 year in a supervisory role in a hospital or home care setting.
- Skills: Familiarity with Patient Review Instrument (PRI) assessments is preferred.
- Additional Requirements: Valid drivers license, proficiency in Microsoft Office, and bilingual skills are preferred.
