Position Details
Family Health West is looking for a passionate PT to join our growing rehab team. The inpatient rehab department is hiring a Physical Therapist who is skilled in acute care and inpatient rehab for patient needs while admitted for rehab services before discharge home.
Family Health West has a robust rehab program for inpatient, outpatient, and pediatric care. We are a rural-based critical access hospital serving a wide variety of patients across western Colorado. Our inpatient rehab department has a comprehensive team of therapists within the PT, OT, and SLP disciplines with an experienced staff who are serious about patient-centered care and great outcomes along with excellent customer service. Candidates for this position can expect a team-based atmosphere and clinical autonomy. We have great relationships with our multidisciplinary teams including nursing, providers, and social work.
Inpatient therapy workflows include 45-minute visits one on one with your patients. Along with support from rehab aide staff and a friendly and courteous nursing team to meet our patient’s needs. ESSENTIAL FUNCTIONS:
- Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
- Communicate necessary information to others as appropriate.
- Evaluate and treat acute care and swing bed patients as prescribed by a licensed physician, with a wide spectrum of diagnoses.
- Identify and initiate appropriate physical therapy interventions.
- Assess and document the effectiveness of treatment and modify the treatment plan to achieve goals.
- Assess educational needs of the patient, family or caregiver and provide education to meet those needs.
- Provide care with the consideration of the physician’s orders and established plan of care.
- Communicate with patient, family, caregivers and members of the healthcare team to promote maximum benefit of care.
- Work cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
- Document objective measures of patient status, within the limits of his/her licensure and ability, as appropriate, in providing care and to assist in assessing the effectiveness of care.
- Communicate the goals and treatment plans of shared patients with the Physical Therapist Assistant, while delegating appropriately, to promote maximum benefits from therapy.
- Complete all documentation required to support the services provided and meet organizational, clinical and reimbursement requirements.
- Maintain positive relations withal customers and family members.
- Ensure that patient charges are accurate and entered within 24 hours of service.
- Complete required documentation, including progress notes, daily notes, discharge notes, and insurance forms within 24 hours of service. Complete patient evaluations within 24 hours of service.
- Contribute to the cost effectiveness of services and programs provided by the department.
- Participate in personal performance improvement and program development activities including administrative staff meetings.
- Communicate observations and suggestions related to opportunities for improvement to supervisor.
- Orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual as requested.
- Assist with administrative duties, including but not limited to answering phones, billing, photocopying and clean up, as necessary for the overall department function when necessary.
- Other job related duties as assigned.
EDUCATION:
1. | Education | Bachelor’s degree (B.A.), Master’s degree (M.A.) or Doctoral degree or equivalent from an accredited college or university with a program accredited by the American Physical Therapy Association (APTA). |
2. | Licenses | Current State of Colorado Physical Therapy license in good standing. |
3. | Certifications | Current BLS certification. |
4. | Experience | No experience necessary, new grads encouraged to apply. |
WAGE RANGE: This position starts at $37.00 and goes up based on experience.
