About the Job
A. SUMMARY:
To enhance the health and wellness of HIV-positive and infectious disease individuals in the community by providing affordable, high quality health care, patient education, support, and advocacy. To provide health education and support to patients to maintain health and wellness, manage acute and chronic illness, and adhere to medications and treatment plans. To provide clinical support to Medical Director.
B. PRIMARY RESPONSIBILITIES:
- Knowledge and understanding of Employee Handbook.
- Carry out the meaning of the CSMD Mission and Vision statements.
- Comply with CSMD Ryan White (RW) policies and protocols
- Comply with RW training requirements
- Practice a culture of safety to reduce or prevent risk of injury, claims, loss, or liability by utilizing the Risk Management and Infection Control Plan.
- Perform responsibilities with a high-quality standard to yield improved compliance, quality, and patient outcome measures by adhering to the Quality Improvement Plan.
- Be present and ready for work as scheduled.
- Attend in-service trainings, departmental meetings, and community events during and outside of clinic operation hours.
- Work cohesively with team members.
- Consults with the Chief Medical Officer, Chief Executive Officer or designee before decisions are made on questions of interpretation.
- Assist in developing, implementing, and evaluating the center's annual health care plan.
- Provide for organization and maintenance of an effective system of medical care with emphasis on wellness and prevention of illness.
- Assist in preparation of monthly reports to Chief Medical Officer outlining the Center's medical productivity and performance improvement adherence.
- Provide an effective communication link between other staff/employees and employees of the medical department.
- Recommends budget items including patient care equipment and supplies to the Chief, Chief Financial Officer and Chief Executive Officer.
- Performs studies/analysis to determine impact for:
- Healthcare outcomes.
- Policy change positives or negatives.
- Departmental efficiencies
- Other areas, as requested by the Chief Medical Officer and/or Chief Executive Officer to assist in administrative decision-making.
- Lead and motivate employees to do their jobs effectively and efficiently.
- Lead by example holding self to the standards of conduct and performance that they command of other medical staff members.
- Attend required organizational meetings and participation on internal committees.
- Meet or exceed annual productivity standards.
- Utilizes technology to create a more efficient and effective way of doing business.
- Maintain confidentiality of workplace information according to the policies and procedures of organization.
- Assist in conducting community outreach to business, community, social and religious organizations.
- Works in close partnership with all Ryan White and Infectious Disease Program staff to provide an affirmative, inclusive, and respectful environment for all patients, with a focus on persons living with HIV.
- Conducts medications adherence visits for patients living with HIV, Hepatitis C and pre-exposure prophylaxis (PrEP) follow-up.
- Receives internal and external referrals for HIV prevention and care, Hepatitis C, and Infectious Disease health care.
- Performs HIV and STI education and testing at all CSMD locations
- Addresses medication refills, medication prior authorizations, releases of information, copying and faxing patient related documents.
- Responds to patient phone calls, text messages and clinic walk-in requests and communicates necessary information to relevant staff member.
- Works with Adherence Counselor to reduce barriers to HIV care and increase access to essential services such as housing, transportation, and legal aid.
- Develops relationships with HIV positive and infectious disease patients and support networks that are culturally competent and sensitive.
- Builds awareness among HIV and other infectious disease patients of health care services and community resources.
- Supports transgender patients with hormone therapy and navigating gender surgery appointments and requirements.
- Helps patients and health care team to identify and address barriers to care including misinformation and/or perceived stigma and discrimination.
- Conduct HIV treatment/ prevention, STI, and Hepatitis C or related organizational training as required or requested
- Participate in departmental research activities
- Represents CSMD at public meetings.
- Represents CSMD at community events such as AIDS Walk, etc.
- Perform other duties as assigned by Chief Medical Officer and/ or the Chief Executive Officer.
C. REQUIREMENTS:
1. EDUCATION:
The FNP is required to be a Registered Nurse with a Master's Degree in the specialty area of family practice from an approved and accredited college or university.
2. LICENSES, REGISTRATION AND/OR CERTIFICATIONS REQUIRED:
Board certified or Board eligible. NP must be licensed to Practice Medicine in the State of Louisiana and possess CDS and DEA licenses. Minimum of 3 years working as an FNP with specific focus on infectious disease.
3. JOB KNOWLEDGE:
The Nurse Practitioner must have the skills and knowledge for budgets and organizational structure and three to five years' proven experience in a FQHC setting; infectious disease preferred. Person must have the ability to direct and supervise skilled and semi-skilled employees.
