Overview
The IL-RN Home Health Care is a critical role within our organization that provides an exceptional level of healthcare to patients in their homes. These are adults facing various health challenges. You will be trained extensively to provide a comprehensive suite of services designed to be done in the comfort of patients home.

Key Responsibilities
Conduct comprehensive patient assessments and evaluations.
Develop and implement individualized care plans.
Provide skilled nursing care in accordance with established policies and procedures.
Administer medications and monitor patient responses.
Perform wound care and other therapeutic treatments.
Educate patients and family members on health management and disease prevention.
Collaborate with healthcare professionals to ensure coordinated care.
Maintain accurate and up-to-date patient records and documentation.
Monitor vital signs and report any significant changes in condition.
Assist in patient mobility and daily living activities.
Provide emotional support and counseling for patients and families.
Participate in staff meetings and contribute to care team discussions.
Assist in the training and orientation of new staff members.
Ensure compliance with ethical and legal standards in patient care.
Promote a positive and inclusive environment for patients and coworkers.
Required Qualifications
Current Registered Nurse (RN) license in Illinois.
NCLEX Review Support provided by the AuffantReview.Com
Available to speak, read, and write in both Spanish and English.
Bachelor's degree in Nursing (BSN) preferred.
Recent Graduates are Welcome.
Certification in BLS (Basic Life Support).
Willingness to travel to patients' homes as required.
Ability to lift and assist patients as needed.
Commitment to ongoing professional development and training.
