The position of Temporary Housing Case Manager will be responsible for maintaining a caseload of residents where Individual Service Plans are developed. Additionally, they will provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of the residents’ needs.  

QUALIFICATIONS:

Minimum Qualifications:

Completion of Bachelor’s Degree in Human Services or related field and a minimum of six months experience in social services.  Communicate effectively in oral and written form with residents, family members, agency staff, other social service agencies, and the general public; as well, strong active listening and cognitive skills.  Ability to gather, collate and classify information about data, people or things; interpret variety of instructions in written or oral format; deal with problems involving several variables within unfamiliar context; write meaningful, concise and accurate reports, define problems, collect data, establish facts and draw valid conclusions; counsel on sensitive matters.  Basic knowledge and understanding of the challenges facing those experiencing homelessness.  Valid Ohio driver’s license and proof of insurance.

DUTIES AND RESPONSIBILITIES:

1. Meet with assigned adult resident within 48 hours of arrival at the Emergency Shelter.
2. Develop Individual Service Plan for each resident within 48 hours and assist residents to achieve goals outlined in their Individual Service Plan.  Objective is for resident to identify, obtain and sustain independent housing.
3. Maintain resident files, case notes, agency forms and other documents within assigned deadlines.
4. Gather personal items from shelter closets including clothing, shoes, coats and any other personal items to meet the needs of the resident.
5. Collaborate with the Health and Wellness Coordinator and Child Advocate to prioritize the clients’ needs for in-house training and education, as well as, identify outside resources to maximize the success of their “ISP.”
6.  Collaborate with community resources to assist residents, i.e. Fair Housing, Legal Aid, Core, Counseling, etc.
7. Assist resident in identifying community and personal resources and discuss the programs available to them.
8. Attend staff meetings as scheduled.
9. Deliver any required reports on a timely basis.
10. Represent ACCESS to community resources as required.
11. Maintain case notes to document resident interaction and submit weekly updates.
12. Act as the Ohio Benefit Bank Counselor and complete the necessary paperwork to sign residents up for benefits as needed.
13. Responsible for adhering to all policy and procedures as outlined in the employee handbook and other departmental procedure manuals.
14. Assist case manager in adding intakes into HMIS, updating resident photo sheet and providing HMIS reports as needed.
15. Making corrections to HMIS Data Completeness Report monthly.
16. Responsible for other duties as assigned by the Director of Client Services.

ACCOUNTABILITY: The Case Manager position reports directly to the Director of Client Services.

• While performing the duties of this job, the employee is regularly required to talk and hear.
• The employee frequently is required to sit.
• The employee is occasionally required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl.
• The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception, and ability to adjust focus.
• The employee must be able to successfully complete CPR certification. 
• While performing the duties of this job, the employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
