Position Summary:
D evelops and meets the agency's needs within the in-home hospice nursing care program. The Clinical RN Supervisor is responsible for coordination, development, implementation and evaluation of the day-to-day care needs of the in-home hospice nursing program including:
- Provides leadership, direct supervision and support to "assigned clinical staff
- Supports good communication and team work among interdisciplinary members of the team.
- Directs, coordinates and dispatches clinical staff for patient care needs.
- Problem solves in relation to patients or the agency in coordination with appropriate team members.
- Conducts utilization review in conjunction with the Medical Director, Director of Clinical Services, and Hospice Director.
- Provides supervision and evaluates assigned nursing staff
- Current unrestricted Washington State Registered Nurse's License
- Associate Degree Graduate of an accredited school of nursing
- Minimum four (4) years of post-education clinical experience
- Certification in Hospice & Palliative Nursing preferred or must be attained within three (3) years of employment.
- Bachelors Degree in Nursing
- Experience in program development, coordination, and management
- Experience in oncology, home health, or hospice nursing preferred
- Two (2) years management and or supervisory experience.
- Bi-lingual
- Demonstrated knowledge of professional standards of nursing practice
- Excellent written and verbal communication skills
- Excellent critical thinking and problem-solving skills
- Confidentiality and HIPAA competency
- Skills to self-direct, and provide direct supervision of assigned staff
- Ability to work independently or as part of a team
- Strong leadership and mentoring skills
- Ability to delegate and assign work
Direct Supervision :
- Coordinate and supervise the planning and delivery of quality, cost-effective hospice care
- Ensure completion and coordination of tasks assumed by other clinical staff
- Advise and support nurses with symptom management challenges;
- Act as a resource for nursing personnel in the assessment of patient/family's knowledge of their disease and their care.
- Supervise the quality of care provided
- Nursing orientation, scheduling, education, and support
- Schedule and facilitate the meetings of the interdisciplinary team.
- Evaluate patient services provided by the interdisciplinary team.
- Measure and track staff productivity.
- Schedule, facilitate, and maintain record of regular staff meetings.
- Act as liaison between Hospice staff, patients, families and other care providers
- Contribute to effective patient and customer relationships by assisting patients, families, providers and staff to resolve expressed concerns.
- Make at least one supervisory home visit with each assigned RN Case Manager each year;
- Approve electronic time sheets;
- Provide yearly written and verbal evaluations for each RN or LPN on the assigned team.
Compliance:
Director of Clinical Services (DCS) :
- Maintain knowledge of state licensure, federal certification, and accreditation requirements.
- Monitor delivery of care and documentation of case managers to ensure compliance with policies and procedures and Medicare/Medicaid conditions of participation
- Adhere to the practice of confidentiality regarding patients, families, staff and the organization;
- Support QAPI, clinical leadership decision making and policy and procedure development by participation on the Clinical Leadership Team (CLT).
- Maintain knowledge of CMS Conditions of Participation and regulatory requirements.
- Assist with utilization review for appropriate use of agency resources;
- Serve as liaison between the medical director or designee, Director of Clinical Services and IDT regarding complicated clinical care issues;
- Facilitate interdepartmental communication with co-workers and staff when appropriate.
- Facilitate, advocate and validate the role of the volunteer as part of the interdisciplinary team.
- Serve as liaison between Director of Clinical Services, Medical Director/Hospice regarding clinical problems and progress in patient care.
- Communicate with the DCS regarding personnel issues, patient care concerns, and utilization of resources.
- Participate in the hiring, orienting and evaluation of new nurses.
- Other tasks as assigned by the DCS
Related Duties and Responsibilities:
- Participate in and present nursing continuing education.
- Complete all required agency annual reviews and competencies.
- Maintain competencies in pain management, symptom management, infusion care, wound care, nursing assessment, medications, and charting in electronic medical record.
- Assist with coordinating the development of the clinical component of patient care policies and procedures.
- Participates in the on-call nursing supervisor rotation schedule. Follows through and completes the on-call supervisor duties. Is prepared and ready and able to step into direct patient care as needed.
- Other duties as assigned.
