General Purpose:
Responsible for providing personal care services to hospice patients, as well as support in incidental/instrumental activities of daily living under the supervision of a Registered Nurse (RN) and in accordance with the policies and procedures of the Interim HealthCare Hospice and applicable state and federal law or regulation.
Essential Functions:
- Provides services as stated in each patient’s Hospice Aide care plan.
- Assists patients in safely performing incidental/instrumental activities of daily living.
- Assigned duties may include but are not limited to:
- Providing personal care: bathing, oral hygiene, hair care and skin care.
- Assisting in dressing and undressing patients.
- Planning and preparing meals which may include shopping.
- Assisting in feeding the patient.
- Changing incontinent products.
- Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure.
- Assisting in ambulation, transfers and/or range of motion exercises.
- Keeping the patient's immediate living area clean and orderly: dusting, vacuuming, emptying trash, washing clothes/linens.
- Offering and assisting with toileting and elimination.
- Provides respite for the patient's family/caregiver when appropriate.
- Promotes a safe environment and maintains an unobstructed pathway while providing care.
- Utilizes infection prevention and control measures such as universal precautions, hand washing, and personal protective equipment.
- Recognizes, reports, and records/documents changes in patient condition and safety to patient’s assigned RN.
- Organizes self to carry out visits/shifts and tasks.
- Attends Interdisciplinary Group (IDG) meetings as requested.
- Completes mandatory in-services and provides required documentation to keep employee file current.
- Documents observations, activities and services provided in an accurate, complete and timely manner.
- Participates in the Quality Assessment and Performance Improvement (QAPI) program, as requested.
- Participates in the Hospice office’s emergency preparedness program, as requested.
- Completes other assignments as requested and assigned.
- May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.
Knowledge, Skills & Abilities Required:
- Able to perform and provide evidence of current CPR certification for adults, children and infants.
- Able to effectively communicate in English with all levels of the workforce, both clinical and non-clinical.
- Able to perform and prioritize multiple functions or tasks.
- Able to read and interpret technical instructions related to the care of the patient.
- Able to visually and aurally observe and assess the patient.
- Able to effectively deal with multiple changes based on patient needs and scheduling.
- Able to provide proof of valid driver's license, if applicable.
- Able to provide proof of valid auto liability insurance if assignment(s) include driving own vehicle to transport patient.
