JOB SUMMARY:
The Homemaker travels to the patient’s/client’s home and provides direct care (Care Level I, which excludes personal care) under nursing supervision in order to safeguard the patient’s/client’s dignity in support of their living independently. The care provided according to a written service plan may include light housekeeping, shopping, meal preparation, laundry, companionship, and assistance with other activities of daily life (not requiring hands on tasks) to assure a safe, clean home environment and the well-being of the patient/client. The Homemaker reports to the appropriate supervisory staff, has no supervisory responsibilities. The Homemaker has HIPAA restricted access to certain patient/client information, is a non-exempt, hourly per-diem Direct Care employee with no guaranteed minimum number of hours per week.
QUALIFICATIONS:
- Have a high school diploma, GED or an acceptable combination of education and life experience.
- Successful completion of the forty (40) hour certificate training and mandated competency evaluation for Homemakers.
- Have the willingness to travel throughout the service area. This means be able to drive and have a valid driver’s license and auto insurance or have the ability to independently travel on public transportation.
- Be physically able to perform the essential functions and duties of the job, read, write, follow written and verbal directions in English, and maintain required records.
- Maturity and dependability are required.
RESPONSIBILITIES:
Drive to patient’s/client’s home and provide the tasks identified on the written service plan. These activities require a variety of physical demands, including but not limited to those physical traits and tasks outlined in Working Conditions below, reliable attendance, punctuality, and the ability to read and interpret the patient’s/client’s written service plan.
- Perform light housekeeping duties such as, dusting, laundry, vacuuming, general cleaning of bathroom, kitchen, and living area when part of the written service plan. Such activities include the use of a wide variety of equipment and home appliances.
- Assist patient/client or perform meal planning, shopping, and meal preparation. Clean kitchen area, dishes, appliances, and work area after preparation.
- Observe patient’s/client’s condition or living arrangements and report changes appropriately. This activity requires good observation skills.
- Provide companionship. This activity requires a caring attitude, courtesy, and patience.
- Complete Time Log and Daily Activity Report accurately and submit on time.
- Maintain and protect patient/client and agency confidentiality.
- Adhere to agency policies and procedures.
- Maintain valid driver’s license and auto insurance.
- Maintain a professional image, good appearance, and personal hygiene.
- Maintain patient/client and agency confidentiality.
- Be punctual and maintain an excellent attendance record.
- Attend/complete agency meetings and training.
- Perform other duties as assigned.
WORKING ENVIRONMENT:
Work is in a variety of home environments. Frequent travel by car or public transportation throughout the service area is necessary. Tasks may involve exposure to blood, body fluids, or tissue (OSHA Category I) and household chemicals, dust, and disinfectants. This position routinely requires driving a car or independently using public transportation, lifting, bending, reaching, kneeling, pushing and pulling, stretching, standing, stooping, walking, walking up and down stairs, seeing, hearing, speaking, writing, reading, carrying, weight bearing activities, and the use of a wide assortment of large and small home appliances.

LIFTING REQUIREMENTS:
Ability to perform the following tasks if necessary:
· Ability to participate in physical activity.
· Ability to work for extended period of time while standing and being involved in physical activity.

· Ability to do extensive bending, lifting and standing on a regular basis.
